Permission for participation and the photo release form MUST be signed by a Parent or Guardian and either dropped off at the counseling office or scanned and sent to cbcyouthreps@gmail.com.

To apply print the application and the photo release form, fill it out, have it signed, and either drop it off at the Meridian High School Counseling Office, scan and send it to cbcyouthreps@gmail.com or mail it to P.O. Box 632, Falls Church, VA 22040.
Please note your application will only be available to and reviewed by the CBC Youth Representative Application Reviewers.
